The One-Minute Discharge: How VectorCare Is Redefining Care Coordination inside the EHR

By embedding VectorCare’s platform within the EHR, care teams can now manage the entire continuum from hospital to home without leaving their existing workflow

VectorCare, a healthcare logistics platform transforming how care transitions are coordinated, has taken a major step forward with the launch of its SMART on FHIR app bringing its scheduling, communication, and care coordination tools directly into leading EHR systems like Epic. This integration marks a significant advance in interoperability, enabling hospitals to streamline discharge workflows, reduce administrative burden, and enhance patient throughput. By embedding VectorCare’s platform within the EHR, care teams can now manage the entire continuum from hospital to home without leaving their existing workflow.

In this exclusive MedTech Spectrum interview, David Emanuel, CEO of VectorCare, discusses the inspiration behind this move toward direct EHR integration, the unique capabilities of VectorCare’s SMART on FHIR implementation, and how this innovation is helping hospitals accelerate care delivery, improve patient experience, and build smarter, more connected health systems.

What inspired this move toward direct EHR integration?

We wanted to meet care teams where they already work — inside Epic. Our new SMART on FHIR app brings all the core functions of VectorCare—scheduling, messaging, and real-time updates—directly into the EHR. This eliminates duplicate entry and phone calls, helping turn a 31-minute discharge process into less than one minute, freeing case managers to focus on patients instead of admin work.

What makes VectorCare’s SMART on FHIR implementation unique?

Unlike basic interoperability tools, VectorCare delivers a full logistics platform inside the EHR. It scales easily across Epic, Cerner, and Allscripts using SMART on FHIR standards, while maintaining enterprise-grade security and compliance. Hospitals gain a single, secure workflow for all service requests—from ambulance to home health—without leaving their EHR.

How does this time reduction translate into measurable benefits?

Saving 30 minutes per discharge helps hospitals free up beds faster, improve throughput, and reduce length of stay. Case managers reclaim hours each day, while patients benefit by getting home or to the next level of care sooner—the right care at the right time.

How does this improve the patient journey?

By connecting the hospital, transportation, and post-acute network in real time, we create a continuous care experience. Patients and caregivers get instant updates on transport and care coordination, while hospitals maintain full visibility from discharge to home recovery.

How are hospitals responding so far?

Very good, and over the next several weeks, we will be announcing partnerships that healthcare systems will benefit from.

What’s next for the app?

We’re expanding with automation and predictive analytics to help hospitals anticipate demand, select the right vendors automatically, and further optimize patient flow. The goal remains the same: ensure every patient gets the right care, at the right time, with zero friction. Standby for further announcements around partnerships.
